I am looking for information on the Big Blue Crane accident at Milwaukee Stadium when they were building it on July 14, 1999. I am trying to find the OSHA report on the accident but I'm unable to find it. Does anyone know where I can find the OSHA report and/or where I can find good free websites about the accdietn?

Does anyone one know if OSHA investigates a fatal accidents on the workplace when there is a violation of saftey regulations or do they investigate all accidents.

If I have worked for years in a hazardous environment?

I work on the 1st floor in a mall, and the backroom of the store is under a photo developing shop. For some reason, the pipes that carry the developing chemicals has leaked and the liquid has spilled all over tons of boxes and its smells terrible. This happened two weeks ago and they still haven't cleaned anything up because all of our holiday shipment is in the way. I used to work in Target developing photos and I know how hazardous these chemicals can be. If I go into work tomorrow and find out that they still haven't cleaned anything up, can I refuse to work even though I work on the floor and not in the backroom? Should I contact OSHA? Many of my coworkers have been in the back breathing in the fumes. I know it's not going to smell as much anymore but I am uncomfortable with being around it even if it's just in the back. What should I do? It's been a mess for over two weeks now and all they tell me is that "they're working on it". Does this seem like the management is handling it properly?

I work at a military base as a civilian cook. Recently the mess hall manager [civilian ] changed everyone's schedule, cut cook positions, , added more work load for everyone. I am trying to find out if his actions violat any OSHA standards, or and is in fact a real safety issue.None of the military cooks there are affected, as they virtually do nothing anyway, it is up to the civilian cooks to do the cooking, serving , cleaning, meal prep. etc. There used to be 3 civilian cooks that came in at 3:30AM , to do a breakfast for 250 military personnel at 5 AM.Due to the cutting of positions, now only 1 cook comes in at 3:30 am, to do the entire meal, plus set up the line, cook the food, serve the food. He is alone in the mess hall , working around potentially dangerous cooking equipment , stoves, kettles, steamers, hot serving lines, knives etc. If he were to hurt himself , no one would know till the line opened at 5 AM, and another worker shows up. The potential for serious injury, is a real hazard , as he is alone, so no other person, civilian or military would know of it for potentially 1 1/2 hours. Does this violate OSHA safety standards, and where can I file a complaint. Thank you for any help you may provide.

i was in a monthly safety meeting when the instructor told a story about a previous employee horseplaying around and caused a back injury to another employee. the instructor stated that this was non reportable because it happened during break .
